Please complete the permission slip below and return it to school by Monday 31^st August so that teams can be entered on time.

PLEASE DON’T SEND IN ANY MONEY UNTIL I AM SURE THAT I CAN ACCOMDATE YOUR CHILD IN A TEAM.
